Temp-to-hire Community Health Worker, Chicago, IL
Our client, an ACO located on the near west side of Chicago, is seeking to add to their team two Community Health Workers (CHW) on a temp-to-hire, full-time basis. This position is a hybrid position with three days in office and two days at home. The CHWs are part of a multi-person team that is involved with screening community members with chronic illness for social determinants of health and provide a suite of comprehensive services to resolve identified issues.

Duties of CHWs:
- Completes Health Risk Assessments (HRA) with patients living in target zip codes.
- Assists with community resources and scheduling appointments.
- Enrolls eligible patients in disease management program(s) and provide disease specific and preventive care patient education according to program requirements.
- Monitors patients (in person or by telephone) at required frequencies and tracks clinical outcomes such as PHQ-9, blood pressure, HbA1c.
- Uses a worklist to identify and re-engage patients who are not participating as expected in the disease management program.
- Supports the treatment plan prescribed by PCPs, focusing on treatment adherence, side effects, other complications, and effectiveness of treatment.
- Facilitates treatment plan changes for patients who are not improving as expected in w/ PCP
- Prepares for and participates in regularly scheduled caseload oversight with the behavioral health specialist and/or oversight nurse
- Engages relevant care management staff with patient updates and ensures the care plan is consistently updated and integrated with disease management information.
- Accurately documents patient information, assessments, interventions and encounter.
- Documentation is done in MHNConnect Portal.
Position Requirements/Knowledge & Skills:
- High School diploma or equivalency preferred
- Highly proficient with use of computers and must be proficient in MS Office products
- Minimum of 2 years’ experience in healthcare outreach
- High degree of creativity & problem-solving
- Ability and patience to navigate and impact complex systems of care
- Ability to develop & employ effective customer relationships w/patients and healthcare team
- Ability to work independently & also as a team with a wide variety of individuals.
- Bilingual Spanish/English is a plus, but not a requirement.
