Temp-to-hire Quality Outreach Associate, Chicago, IL
Our client, an ACO, located on the near west side of Chicago, is seeking to add to their team another Quality Outreach Associate on a temp-to-hire, full time basis. This position is a hybrid position with three days in office and two days at home. The Quality Outreach Associate is part of a multi person team that works to improve patient health outcomes through education and outreach.

Duties of Quality Outreach Associate:
- Review and address open care gaps
- Outreach to members and/or providers regarding recommended health screenings
- Assist with appointment/transportation scheduling
- Attend off-site meetings at provider offices to retrieve medical records
- Conduct Medical Record review/Chart scrub
- Validate care gap closures from completed appointments
- Build and maintain relationship with physicians and support staff
- Provides support to providers and support staff with open care gaps
Position Requirements/Knowledge & Skills
- High School diploma or equivalency, associate degree preferred
- Knowledge of Electronic Medical Record Systems (EPIC, NextGen, E Clinical, Practice Fusion)
- Minimum of 2 years’ experience in healthcare
- Intermediate proficiency in Microsoft Office applications
- Intermediate proficiency in Sales Force
- Knowledge of NCQA Guidelines for HEDIS measures, patient quality, and CPT codes
- Strong Communication and Interpersonal skills, bilingual a plus
- Appointment Scheduling Experience
- Hospital/Medical Office Experience, knowledge of Medical Terminology
- Telephone Skills, logging calls and tracking results
