Age Specific Responsibility
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in assigned department. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to age-specific needs and to provide the care needed as described in the department’s policies and procedures.
Summary
The Clinical Care Coordinator (CCC) for Mary Free Bed at Covenant is responsible for the overall operation of the rehabilitation hospital and Transitional Care Unit (TCU), and has 24 hour accountability for all patient care activities and staff who work under his / her direction. The CCC is responsible to organize, direct, and ensure effective operations of the rehabilitation hospital and TCU for the assigned shift, taking into account the effect of decisions on the 24 hour operation of the units.
The CCC participates with managers in hiring, providing corrective action, and completing formal evaluations of employees. Each CCC uses independent judgement in problem solving, takes necessary steps to resolve issues, organizes work flow, assigns employees and participates in budget planning and compliance. This leader maintains clinical competency and participates in direct patient care on an episodic bases to assist with staffing in times of high census, as a consultant to staff, or in response to emergency situations.
The IP Rehab / TCU Clinical Care Coordinator is responsible to demonstrate excellent customer service, while upholding the Mary Free Bed Standards of Behavior, and supporting the mission, vision and values of both Mary Free Bed and Covenant HealthCare.
Essential Job Responsibilities
Quality Pillar of Excellence
· Quality Nursing Care – The Clinical Care Coordinators’ primary focus will be the delivery of quality nursing care. This includes working with all staff to assure that nursing care is delivered consistently according to established policies, procedures, and protocols including activities such as creating equitable staffing assignments, mentoring new staff, ensuring nursing presence at and preparedness for team conferences, communication to and follow up with physicians, safety monitors, and timely follow up on occurrence reports as well as customer service recovery as needed.
o The Clinical Care Coordinator is responsible to review and provide input to the Nurse Manager for nursing policies and procedures; nursing standards of patient care, treatment, and services; and standards of nursing practice. The Clinical Care Coordinator will be responsible for addressing non-compliance with policies, procedures, and validation of key nursing tactics.
· Key Performance Indicators – The Clinical Care Coordinator is responsible to understand performance measures within the Rehabilitation Hospital, and to collaborate with staff to identify opportunities for improvement and potential action plans. When process, policy, or procedure changes are developed by the Program Director or Nurse Manager, the Clinical Care Coordinator is responsible for directing staff to the information, assist with implementation of the changes, working with staff to complete competencies as needed, and providing feedback to the Nurse Manager. Clinical Care Coordinator will participate in coaching under as requested by the Nurse Manager.
· Quality Improvement – The Clinical Care Coordinator will participate in Mary Free Bed at Covenant’s quality improvement program. The Clinical Care Coordinator will work with staff to provide excellent rehabilitation nursing care through innovative ideas that align with evidence-based policies and procedures.
· Patient Safety – The Clinical Care Coordinator plays an important role in providing a safe environment for all patients and staff. As a clinical resource for employees, the Clinical Care Coordinator will evaluate the risks for each patient and as a patient advocate, assure that appropriate steps are taken to protect the safety of each patient. The Clinical Care Coordinator will also serve as the primary clinical support for MFB employees throughout the hospital and responds to all Medical Emergencies calls, per policy as well as all codes (Fire, Tornado, Violence, Abductions) within the campus.
· Interdepartmental and Interdisciplinary Collaboration – The Clinical Care Coordinator is responsible to maintain effective interpersonal communication and working relationships with all leaders, staff, physicians, and other key stakeholders. As assigned, the Clinical Care Coordinator will participate actively in interdisciplinary program meetings with the goal of evaluating and improving the care provided at Mary Free Bed at Covenant.
· Accreditation and Regulatory Responsibilities –The Clinical Care Coordinator assists the Nurse Manager and the Program Director with implementing policies, procedures, and processes that align with regulatory, accreditation, and state hospital standards. The Clinical Care Coordinator will serve as a resource for staff in these areas.

· Customer Service – Each Clinical Care Coordinator is required to demonstrate excellent customer service standards and behaviors, while interacting with patients, family members, physicians, and colleagues. The Mary Free Bed Standards of Behavior outlines expectations for performance in this area. Each CCC must deal discreetly and sensitively with confidential information, and as needed, will perform customer service recovery with patients and families members. The CCC will also take active steps to assist staff members to recognize unique cultural and religious needs of patients and families and integrate these diverse needs into a patient-specific plan of care that aims to exceed the expectations of each patient and family.
· Physician and colleague relationships – Interdisciplinary team integration and collaboration across disciplines to provide patient-centered care is a priority at Mary Free Bed. Maintaining effective working relationships with admitting and consulting physicians is an important responsibility of the CCC. Each CCC is responsible to develop and maintain professional working relationships with colleagues from other disciplines that are involved in the care of each patient and family. When challenging situations are identified by staff members, the CCC will assist with problem solving and service recovery, as appropriate.

· Employee Competency – Each CCC employed at Mary Free Bed at Covenant is responsible to perform within the policies, procedures, and standards of care set by the organization. This often requires demonstration of competency to meet regulatory and accreditation requirements and in alignment with recognized standards of care and best practice benchmarks. Each CCC is responsible to demonstrate competency in those areas required by the department, within the defined time frame. Additionally, the CCCs will assist the Nurse Manager and/or Program Director with assessment of competencies for new and existing staff members, and will share concerns promptly when they are identified.

When risks to employee safety are identified (self or others), each CCC will take appropriate steps to reduce those risks, communicate with the staff, and to maintain a physical work environment that reflects the high quality nursing services delivered at Mary Free Bed at Covenant.
· Staff Selection, Development and Mentoring – As requested by the Nurse Manager, the Clinical Care Coordinator will assist with employee selection and the orientation process for new team members. Interview questions and feedback will be presented professionally and tactfully, and information discussed during the interviews will remain confidential, if needed. The Clinical Care Coordinator will maintain open communication with the RN preceptors and the Nurse Educator to provide a comprehensive orientation for the new team member.
· Student Preceptor and Mentoring – The CCC will assign RN preceptors for students and orientees, in alignment with the education plan developed by the Nurse Educator. The CCC will oversee the care on his or her shift to assure that a valuable clinical rotation is provided for students.

Customer Service Responsibilities

Essential Job Qualifications
· Licensure: Registered Nurse in the state of Michigan.
· Education: Associates degree in nursing is required. Bachelor’s degree preferred. Certification as a Registered Rehabilitation Nurse is preferred and will be encouraged for all MFB RNs.
· Current Basic Life Support (BLS) certification
· Experience:
o Previous experience as a RN, especially in a rehabilitation or acute care setting, is preferred but not required.
o Strong critical thinking, decision-making, delegation, and leadership skills are required to work as a CCC at MFB at Covenant.
o Time management and organizational skills are very important, as well as effective written and verbal communication skills.
o Must be able to work effectively under time requirements and deadlines, problem-solve when facing unexpected issues, handle interruptions, and prioritize effectively.
