Summary
The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1.
Education
Bachelors Degree in Social Work or related field required. Masters Degree in Social Work preferred.
Certification, Registration & Licensure
Experience
Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting. Demonstrates effective interpersonal, organizational and motivational skills. Demonstrates ability to function as a team player. Proficient in written and verbal communication.
Responsibilities
Upon consultation, conducts a comprehensive assessment of the patient's behavioral, psychological and
socio-economic needs.
Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance of
the patient and family's goals.
Participates in end of life discussions and advance care planning.
Bases social work actions by code of ethics according to National Association of Social Workers (NASW).
Documents measurement and evaluation of social service consultation and satisfaction outcomes as
patient progresses towards desired goals.
Provides leadership to departmental staff.
Performs other duties as assigned.
