PROLOGUE JOB DESCRIPTION
Position Title: Intake/Benefits Coordinator
General Description: The Intake/Benefits Coordinator screens individuals for admission into the Psychiatric Rehabilitation Program (PRP) and Residential Rehabilitation Program (RRP) (Baltimore & Carroll County). The Intake/Benefits Coordinator works with referrals to obtain consent for services, medical records, and other necessary documentation to admit individuals into the programs. The Intake/Benefits Coordinator also obtains necessary billing authorizations and determines any fees for services. The Intake/Benefits Coordinator develops an initial plan of action and links individuals with their respective rehabilitation team. The Intake/Benefits Coordinator works with the local Department of Human Services to secure entitlements such as Medicaid, Public Assistance to Adults and Supplemental Nutritional Assistance Program. Schedule: Full time. This person reports to the PRP and RRP Program Directors.
Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:
A bachelor's degree with at least one (1) year experience in mental health/psychiatric rehabilitation/related human services is necessary. Must be able to work independently and with a team as necessary. A good driving record is required. Ability to respect individual choice in the recovery process and a commitment to recovery through the psychiatric rehabilitation process is required. This position requires good verbal and written communication skills. The intake coordinator must be organized, detail oriented and adhere to deadlines to provide timely access to referrals.
Summary of Essential Functions, Responsibilities and Duties
- Seek and accept referrals
- Perform initial screening and obtain authorizations
- Complete admission notes and all paperwork with client/resident per COMAR and CARF regulations
- Provide both on and off-site meetings for screenings (state hospitals, county hospitals, community, etc.)
- Meet with hospital team to discuss progress in treatment
- Provide case management services to new clients for 60-days
- Initial appointments, benefit applications, initial ISP, etc.
- Coordinate and assist individuals transitioning in the RRP/PRP
- Meet and greet newly enrolled individuals, introduce to treatment team and provide orientation process
- Monitor progress with rehabilitation team and individual during transition schedule
- Provide initial benefits coordination (SNAP, PAA, SSA, etc.)
- Provide tours to interested referrals and families
- Meet with families to provide resource information
- Complete Food Program Applications
- Submit authorization requests through the Administrative Services Organization (ASO)
- Complete initial Biopsychosocial Assessments on all new individuals being enrolled
- Complete statistics by the 10th of each month (referrals, intakes, screenings)
- Maintain detailed spreadsheet of all referrals with disposition information
- Complete DHS benefits for currently enrolled individuals (PAA, SNAP, MA)
- Provide on- call service for PRP on a rotation basis
- Utilize Credible Electronic Health Record to document services
- Provide on-site coverage as requested
- Provide off-site services to individuals in the program
- Participate in county and state placement/provider meetings
- Conduct marketing to community providers and resources
- Perform other duties as requested by supervisor(s)
