Intake Case Manager

+ The Intake person works with the Operations and Intake Directors and is an immediate contact person for program referrals. They are responsible forconducting a comprehensive intake with the referral, arranging evaluations, treatment admissions, and developing ongoing supports for the care team. Responsibilities and Duties include:• Conduct initial comprehensive intake• Review and staff case with the Clinical Team• Arrange evaluations/treatment• Review evaluation and/or treatment recommendations• Routinely review case during intake staffings• Maintain and follow program policies and procedures• Utilize documentation in our company systemWe need someone with:• Proven case management experience with knowledge and experience with mental health• Excellent verbal and written communication skills are a must• Excellent time management and organizational skills• Experience with Microsoft Office and electronic documentation
_Qualifications:_
+ Bachelor’s degree in human services or related field.
+ One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc. preferred.
+ Current driver's license, car registration, and auto insurance.
+ A reliable, responsible attitude and a compassionate approach.
