Kids Above All is seeking a Intake Coordinator/Case Manager for our Waukegan office.
Job Summary: The Case Manager/Intake Coordinator will perform case management, social service, and recruitment for a caseload of 20 clients. In addition, this position will assist with assuring intakes are conducted on all incoming referrals. This position will assist clients in obtaining support for victims through the State of Illinois. The Case Manager/Intake Coordinator will support clients with connections to other resources, such as health care, legal assistance housing placement, and vocational support and training.
Essential Functions:
- Assist victims with a wide variety of wrap around services including health care, legal assistance, housing placement, etc.
- Assure all referrals to program have an intake to determine if services can be provided by the program or if client is in need of referrals for assistance elsewhere in the community.
- Responsible for recruiting a caseload of 20 clients
- Actively participate in community meetings, network with schools, hospitals, etc.
- Identify scope of community resources and refer families as needed
- Complete data entry of client information as required
- Prepare, maintain, and provide documents and reports as required by the Agency
- Participate in ongoing training and meet all Agency and/or DHS requirements
- Stay current in job and/or client specific competencies; Participate actively in Continuous Quality Improvement initiatives and activities
- Perform other duties as assigned to fulfill the mission of the Department and the Agency
Qualifications:
- Bachelor’s degree in human or Social Services, or related field.
- Minimum of 0-6 months of on the job or internship experience required.
- Driver approval standards, plus access to an insured auto required
- Must have a perceptive awareness of the needs and responses of children, and the capacity to work well with adults
- Flexibility and willingness to work evenings and/or weekend hours, if needed to carry out all job responsibilities
- Basic computer skills and knowledge of Microsoft Word required
Salary Range: $45,000-$55,000
