Job Summar: The intake Coordinator is an extension of the case management department. The purpose of this position is to create a welcoming experience to treatment for new clients. The intake coordinator is responsible for completion of all intake documentation, gathering basic demographic information, reviewing financial, legal, and vocational needs, and providing the client with an orientation to the facility. The intake coordinator is also responsible for providing clients with additional support throughout the clinical day.
Duties/Responsibilities:
Completing all intake documents
Gather and confirm basic information such as marital status, date of birth, social security number, emergency contact information, and insurance information
Conduct the case management admission assessment
Review and explain important admission forms, standard releases of information, financial form, and AOR forms
Complete daily progress notes
Attending interdepartmental morning meetings
Conduct client orientation group sessions and documentation associated with it
Complete all documentation within the assigned timeframes
Assists and completes phone calls with patients to their families upon arrival
Assistance in completing Coordination of Benefits documents
Ensure the patient understands the level of care and treatment expectations
Facilitating patient tours of the treatment facilities
Redirecting and reporting patients if rules are broken
Communicate effectively both orally and in writing
Independently solve problems and follow
Organize and function independently in an office environment
Perform other tasks and duties as assigned
Completes all competency assessments as required by the facility
Education and Experience:
2 years experience working with people with behavioral health needs (trauma, mental illness and/or substance abuse)
Experience in providing services in the mental health or substance abuse community
Experience working on a multidisciplinary team
Tenacity and ability to think outside the box and work creatively to engage participants
Knowledge of the criminal justice system and community resources
Computer literate. Proficient in Microsoft Office applications
Excellent verbal and written communication skills
