The Integrated Behavioral Health Provider works as part of a collaborative treatment team of medical providers, nurses, medical assistants, care managers, and office specialists to provide high-quality, compassionate patient care that supports mind-body-spirit health. This behavioral health provider (BHP) reports to the Primary Care Practice Manager. The BHP works to assess, diagnose, treat, and collaborate with the rest of the treatment team and ensure that the entire team is working in unison to meet patients’ needs. The BHP will work to create unique treatment plans for patients with a range of behavioral health issues, addressing the intersection of physical and behavioral health. The BHP will engage in brief interventions during medical encounters and short term therapy sessions to help patients develop strategies that promote health, working to improve the wellbeing of our entire patient population.  

The best candidates for this position are flexible and team-minded, willing to think creatively outside the bounds of traditional mental health care to help patients, their family members, and their treatment team. The best candidates will be open to ongoing learning about health conditions, evidence-based interventions, and the nuances of the healthcare system. They will be confident when communicating within a multidisciplinary team and skilled at building rapport quickly with patients.

ESSENTIAL FUNCTIONS:

• Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
• Works with the primary care team to effectively treat and manage patients with behavioral health problems, especially those that affect physical health.
• Communicate necessary information to others as appropriate.
• Conducts brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing medical, behavioral, mental health or addiction treatment.
• Engages meaningfully with patients through both brief encounters (10-20 minutes) and longer planned interventions (45-60 minutes), including individual, couple/family, and group sessions.
• Maintains professional connections with community resources to facilitate external referrals when needed and assists with the referral process.
• Develops care plans based on diagnostic assessment information, including symptoms, risk factors, strengths, identified goals, and estimated time frames to meet objectives.
• Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical harm.
• Evaluates and/or revises patient care plans with the primary care team.
• Assists in preventing relapse or morbidity in conditions that tend to recur over time through the use of evidence-based measurement tools.
• Teaches patients, families, and staff about care, prevention, and treatment enhancement techniques.
• Uses data to monitor key performance indicators in the clinic’s behavioral health program, identifying problems related to patient services and team-based care and making recommendations for improvement.
• Attends and participates in meetings, quality improvement activities, and committees as requested.
• Engages in professional development to ensure fidelity to a primary care behavioral health model and deepen understanding of best practices for collaborative care.
• Communicates effectively and demonstrates courteous and cooperative behavior with medical providers, employees, patients, families, and the public. Obtains supervisory intervention to assist in problem solving or maintain work relationships.
• Must be able to adapt and remain productive in an environment that has ongoing interruptions.
• Demonstrates compliance with governmental regulations, including OSHA, HIPAA, and CILA, as well as site-specific policies.
• Completes documentation in a timely manner and in compliance with state, federal, professional, and ethical guidelines.
• Engages in routine consultation with the Primary Care Practice Manager and other members of the integrated care team.
• Other job related duties as assigned.

Wage Scale: This position starts at $30.00 and goes up based on experience.

Experience

• Integrated healthcare or other team-based experience preferred.

Education

• Masters or better in Behavioral Health or related field
