Integrated Care Programs Manager
The Manager of Integrated Care Programs provides care management supervision and roster management, as well as implementation and monitoring of managed care partnerships that seek to integrate health care, behavioral, and social supports with in home services in accordance with contract terms, MassHealth guidelines, and MVES policies.
This is a full-time, non-exempt (35 hour/week) position. We offer a hybrid remote schedule (2 days/week in the office) and the option of a flexible 4-day work week. Our office is located in Malden, MA with ample free parking!
This role plays a crucial part in linking older adults, individuals with disabilities, and their families to Mystic Valley Elder Service's (MVES) services and providing information on community resources, in addition to care planning/case management, and short-term follow-up on care recommendations.

Responsibilities:
Essential functions of the position are below. Additional duties may be assigned as required.
- Provide consultation and serve as a resource for assigned team and to others in the Integrated Care Programs department in the absence of their designated supervisor.
- Ensure appropriate coordination between GSSCs, Senior Care Option plans, other providers, and administrative staff.
- Act as a liaison to the SCO plans on LTSS and clinical issues.
- Ensure that GSSC and in home services are delivered in accordance with contractual requirements and SCO authorizations.
- Work with the finance department to ensure accurate billing for care coordination and community services delivered through the MVES network.
- Monitor case records for accuracy, completeness, and continuity of the consumer's status.
- Ensure that quality assurance and customer service standards are met.
- Encourage staff development through supervision and training.
- Evaluate individual staff performance and hold staff accountable for meeting standards.
- Identify and address any performance/policy issues with direct reports.
- Provide crisis care management support as appropriate.
- Promote and support team model and interdisciplinary care management.
- Participate in the interviewing and selection process for hiring new staff.
- Participate in the orientation and training process for new staff.
- As necessary, act as back up to GSSCs to ensure essential tasks and functions are completed timely during staff transitions and periods of turnover.
- As a member of the supervisory team, assist the Director of Integrated Care Programs in ongoing department planning and development.
- Serve as a liaison with the human service networks in the communities served.
- Participate in task oriented committees.
- Develop and maintain programmatic reports as required.
- Assistant with maintenance of Integrated Care Programs policy and procedure manuals and workflows.
- Coordinate and assist in the development of new initiatives and QI within the Integrated Care Programs team.
Requirements:
- Work based out of the office a minimum of 2 days per week
- Work based out of the office additional days as need when mentoring/shadowing new staff
- Provide manager coverage as needed, in office, to ensure there is always a Team IC manager available during hours the agency is open.
Qualifications:
- Bachelor's degree.
- 3 years of experience in the field of home and community-based services for the elderly and/or people with disabilities.
- Knowledge of and experience working with Masshealth Managed Care Plans (SCO, One Care, ACO, PACE, etc.)
- Knowledge of MassHealth eligibility and application/renewal process
- Ability to collaborate with a multidisciplinary team.

- Excellent communication skills, both written and verbal.
- Strong leadership and managerial skills.
- Ability to effectively facilitate meetings.
- Private transportation.
- Computer skills: Proficient with MS Office Suite, and entering data into a database. Working knowledge of Excel. Ability to use the Internet to conduct information searches.
Preferred Qualifications:
- Supervisory experience.
- Experience working in ASAP/AAA setting
- Social work licensure (LSW).
