Integrated Care Specialist - Centurion Health

Centurion is proud to be the provider of comprehensive healthcare services Indiana Department of Correction. We have partnered with Sondhi Solutions as our subcontractor for transitional healthcare services.

We are currently seeking a full-time Integrated Care Specialist to join our Regional Office team located in Indianapolis, Indiana.

The Integrated Care Specialist develops and implements transitional healthcare plans for specific releases with behavioral health needs. This position requires maintaining detailed data and ongoing spreadsheets with specifics as well as coordinating and working closely with IDOC (Indiana Department of Correction) staff, Centurion transitional healthcare facilitators and liaisons and other state or local agencies to ensure that necessary pre-discharge planning and preparation is accomplished for individuals leaving the prison environment and entering the community. 

• Bachelors’ degree in Psychology, Social Work, Counseling, or related field from an accredited educational institution
• Must have experience with and/or knowledge of community and state programs and services
• Must have experience in re-entry, release planning, transitional healthcare planning
• Minimum of two years’ experience working within the correction setting preferred
• Must maintain current tuberculosis documentation and active CPR certification
• Must be able to pass a background investigation and obtain agency security clearance where applicable
• Must have strong organizational and communication skills
