JOB SUMMARY:
Reporting to the Social Services Lead the Intensive Housing Case Manager provides case management services to chronic homeless people in the community, assisting participants in securing permanent housing, and provides referrals for supportive services to community partners, all with the intent of maximizing self-sufficiency and moving people into housing as quickly as possible. In addition, the Intensive Housing Case Manager will provide support in building life skills and attaining short- and long-term goals.
DUTIES AND RESPONSIBILITIES:
- Performs outreach to encampments, shelters, and other service providers within the metropolitan area.
- Meet regularly with families/individuals, assisting them in identifying resources, developing action plans, and securing services.
- Provide information and appropriate referrals and facilitate access to other resources as needed.
- Once permanently housed, collaboratively develop housing stabilization plans and strategies that create self-sufficiency and promote safety.
- Perform weekly home visits to ensure housing stabilization plan is followed.
- Assist families in establishing goals and working toward them.
- Develop knowledge of community resources, utilize advocacy skills and teach self-advocacy so individuals or families can expand their use of resources in the community.
- Evaluate progress and work on areas of concern. Explore ways to eliminate barriers to success.
- Maintains current case files on each program participant.
- Participate in community collaboration groups and represent the organization to others as requested.
- Facilitate communication and coordination between care/service/support providers.
- Complete all required internal and external reporting accurately and on time.
- Teach practical skills such as budgeting, credit repair, and financial management to improve housing stabilization outcomes.
- Obtain necessary releases and consents and advise the patient about their rights and responsibilities.
- Develop a care management plan to integrate physical and behavioral healthcare needs and establish goals.
- Encourage individuals or families to attend voluntary education and support groups.
- Assist participants with program leases, supplemental contracts, and other agreements.
- Assist in identifying permanent affordable housing/home ownership opportunities.
- Mediate/negotiate with landlords when necessary.
- Assist participants in garnering furniture and other household items.
- Review program participant files and ensure that all program records are maintained as required.
- Record number served and services provided. Compile statistical information on services provided as required. Provide information for input into required database.
- Participate in weekly staff meetings, client review and case conferencing.
- Perform other duties as needed to help effectively operate the program.
- Prepares, manages, and evaluates an appropriate service plan for each client.
- Maintains working relationships and involvement with area hospital and clinic social workers.
- Access and follow-up on referrals to community resources in a timely manner.
- Ensures client charts accurate, complete and legible.
- Ensures submission of required paperwork in a timely manner (log sheets, case openings/closings, Medicaid/Medicare forms, etc.…)
- Adheres to office protocol. (Sign in/out sheets, work hours, voicemail, staff meetings, etc.…)
- Interacts with clients, providers and staff in a professional, courteous and responsible manner.
- Completes 40 hours of continuing education hours training per year.
- Performs other duties as directed by your supervisor.

- Promotes, models, mentors and leads customer service excellence.
- Communicates consistently with Director of Client Services and Housing Specialist.
- Must have the ability to work at least a 40-hour a week schedule with no physical limitations in normal ranges of sitting, standing, walking, talking, and lifting up to 40 pounds.

SUPERVISORY RESPONSIBILITIES:
- None
QUALIFICATIONS:
- Knowledge of Metropolitan area human service systems and agencies.
- Work experience with emergency shelter, transitional housing programs, and/or other residential programs.
- Knowledge and experience with the following: domestic violence, crisis intervention, child abuse, substance abuse, mental health, welfare rights, case management, support group facilitation, sexual assault, employment services, child development, and/or positive parenting skills.

EDUCATION/EXPERIENCE:
- Bachelor’s degree in human services, Social Services or related field and two (2) years’ experience working with homeless families in crisis.
- Understanding of HUD regulations.
- Occasional flexibility in scheduling.
- At least one year direct social service experience with low-income and homeless population and/or emergency assistance program.
- Ability to work well with community agencies, congregations, organizations, volunteers and other resources.
- Ability to assess participant barriers and qualities to success.
- Ability to make sound judgments without on-site supervision.
- Good organizational, record keeping, time management, and communication skills.
- Strong community building skills. Experience, ability, and willingness to communicate and work with staff, volunteers, and program participants from a variety of racial, cultural, lifestyle, and economic backgrounds.
- Ability to work with diverse populations.
- Ability to schedule work production to meet timelines with attention to detail.
LANGUAGE SKILLS:
- Ability to read, write, speak and comprehends written documents fluently.
MATHEMATICAL SKILLS:
- Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY:
- Ability to define problems, collect data, establish facts, and draws valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
CERTIFICATES/LICENSES/REGISTRATION:
- No professional certifications, licenses or registration required. All candidates will be required to submit a criminal background check and drug screening as a part of pre-employment.
WORK ENVIRONMENT:
- A large percentage of the time could be outdoors with exposure to odors, dirt, occasional fumes and the general public.
