Job Title: Nurse Practitioner or Physician Assistant (Primary Care)
Location: Colebrook, NH
Salary: $120k-130k
We have an exciting new full-time permanent opportunity for an experienced APRN (Family Medicine) with one of our leading healthcare facilities in Colebrook, NH. Seeking a Full-time board-certified Advanced Practice Provider to join a well-established team in rural health clinic system in Colebrook, NH and form strong relationships with grateful patients in an underserved area. This position is located in a peaceful outdoor paradise in Northern New Hampshire where there is a cultural emphasis on family, independence and hard work.
Requirements:
Will provide direct patient care services that will include, but not limited to recording physical and mental status, performing physical exams, ordering diagnostic tests, implementing and managing plans of care, as well as prescribing and dispensing medications and more. Direct patient care time will be split between acute care and established patient panel. The provider will have excellent interpersonal and communication skills and ability to maintain a good working relationship with all levels of personnel, medical staff, administration, and the public as well as share the values of the Center.
Primary Care:
o Pediatric through Geriatric
o High elderly population
o Experienced in Chronic Disease Management
o 16-20 patients per day
o 36 patient hours, 4hrs of admin time - NO CALL
o EMR is Meditech Expanse, Dragon dictation
o Supported by an RN Team leader and medical assistant

Requirements:
- Licensed by the State of New Hampshire.
- Demonstrates the ability to perform the technical skills and critical thinking outlined in the performance standards.
- Must have minimum of 2 years in practice in outpatient primary care.
- Base of $129,422
