Internal Medicine - Physician - Mission Pointe Rehabilitation Center of Detroit - LTC Physician Care PC

LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.

LTC is seeking a dedicated B/C Physician to join our team a Part-Time Medical Director/Primary-Care Provider.

Requirements and Qualifications

• Experience in Family Medicine, Internal Medicine, or Geriatrics is strongly preferred.
• Candidates must hold a valid and unrestricted medical license in Michigan.
• Candidates must hold a valid DEA license in the state of Michigan.
• Candidates must be able to satisfactorily complete a pre-employment background screening process (Criminal History, Licensing, etc.)
• Candidates must pass a pre-employment drug-screen

Job Structure

• This is an in-person position
• Providers will commute directly between home and their assigned facility
• Physician will supervise 3-4 APPs
• Physicians will act as Medical Directors in up to 3 facilities
• After-Hours on-call is shared among all LTC Providers (currently a 15-week rotation)
• Standard Monday-Friday 8am-5pm workday
