Position Summary
The Intensive Residential Support Home (IRSH) provides intensive community-based services for individuals with intellectual disabilities (ID), mental health needs, and complex behavioral health supports. Our program works to enhance the quality of life for individuals with ID, mental health needs, and complex behavioral health supports by utilizing positive behavioral supports, multi-model clinical assessments, and other therapeutic tools.
**This position requires regular travel throughout the service area (mileage reimbursed) and on-call responsibilities**
Job Description
· Must be capable of regular and predictable attendance at a specific location to perform assigned tasks. Regular travel is required.
· Responsible for the hiring, orientation, ongoing training, performance evaluation, supervision and any progressive disciplinary actions with employees working within the IRSH assigned program.
· Responsible for developing a staff working schedule for the home and always ensuring adequate staffing ratios. This may include working a shift when needed.
· Provide on-call crisis response and assessment on a rotational basis.
· Responsible for monitoring to ensure the health, safety, welfare, rights, and dignity of persons served in the home.
· Maintain and monitor acuity levels of persons served within the IRSH.
· Actively participate in discharge planning and monitoring.
· Responsible for developing and coordinating the person-centered service plan and providing service coordination for persons served in the home.
· Facilitate structured groups with persons served.
· Ensure the coordination of support meetings and completion of clinical documents, assessments, and other START-specific materials for all persons served.
· Responsible for maintaining open and ongoing communication with case managers, region service coordinators, region personnel, MCOs, DHS quality management personnel, legally responsible persons, persons served, and employees.
· Responsible for ensuring policies and procedures are adhered to in the home.
· Enter, monitor, and ensure all documentation entries are completed according to State and START fidelity measures and/or requirements.
· Work with the Nurse Manager to track and oversee medication management procedures.
· Ensure the home is well maintained, both inside and outside of the home, and is in good repair.
· Responsible for promptly reporting allegations of abuse to the appropriate department and reporting allegations of fraud, waste, or abuse to the Regional IRSH Director.
· Participation in program-specific meetings
· Conduct audits of the IRSH managers, counselors, and homes to ensure State regulations, policies, and procedures are adhered to and met for all documentation and billing purposes along with quality improvement plans.
· Attend, participate, complete, and maintain the Center for START Services Coordinator, medication management, and MANDT certifications.

Qualifications
· A Registered Nurse or bachelor’s degree in Social Work, Psychology, Counseling, or another human service field with 4 years experience working with people who have ID and mental health and/or challenging behavior needs and supervisory experience is acceptable.
