Community Bridges is looking for a full-time Direct Support Professional for our Supported Employment Program in Concord, NH!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The Job Coach has the responsibility to provide on the job assistance to those with barriers to employment. The position includes assessing vocational abilities and safety skills as well as coordination of career activities including; interviewing, filling out job applications, resume writing, assistance with job placement as needed as well as primarily on the job supports. On the job supports include role modeling, problem solving, interpersonal skills, and mentoring the individual to create success on career paths.
Qualifications:
- Education: High School Diploma or GED required.
- Experience: Experience in the human service or related field with persons with disabilities desirable skills in coaching, mentoring, or vocational also desirable
Reliable transportation, a valid driver's license, and proof of automobile insurance.
Compensation and Benefits:
- There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.
- Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

- Part time: Monday through Friday with flexible hours (20 hours)
Specific Duties and Responsibilities:
- Support individuals on the job site ensuring proper mentoring and success in the role based on the specific job duties the individual will be performing.
- Promote independence with job functions and provide skills training for how employees perform successfully on the job while modeling professional behavior in the workplace.
- Help design plans and work with employee's onsite to create a successful fade program and be a resource to the job site for ongoing support.
- Provide supports while considering the preferences of individuals. Actively encourage choice making.
- Assist in developing the Individual Service Plan, provide information for assessments, develop implementation strategies for goals/objectives, and monitor the delivery and quality of supports.
- Complete all necessary documentation as specified by both the department handbook as well as the Individual Service Plan. These include but are not limited to, monthly Progress notes, daily AWARDS notes, incident reports, data collections, probes, and individual data.
- Demonstrate an awareness of and respect for each person's legal, civil, and human rights- including maintaining confidentiality of information regarding the individual. Ensuring the respect and dignity of persons with developmental disabilities as well as reporting any safety, health or human rights concerns immediately to the Supervisor.
- Attend team meetings, agency meetings, trainings and seminars as assigned.
- Perform all delegated responsibilities, which may include filing, updating data collection tasks, and other activities related to the provision of supports and services.
- Perform other duties and responsibilities as assigned.
Required Skills:
- Good interpersonal skills including confidence and the ability to initiate appropriate contacts with business representatives.
- Ability to create supports on the job site and provide on-going support while maintaining a positive attitude in all job settings.
- Effectively teaching skill competencies to facilitate job readiness to individuals that can allow them to progress in independence on the job.
- Good verbal/written communication skills in group and one-to-one settings as well as ability to take direction and think creatively.
- Ability to advocate for, provide support to, and assist individuals to advocate for their rights.
- Ability to collaborate with families, team members and community citizens on behalf of each individual.
- Ability to successfully complete and maintain First Aid, CPR, MOAB and additional trainings as assigned.
- Demonstrable knowledge of Community Choices mission, values, policy and procedures.
- Ability to acquire computer competencies.
Work Environment and Physical Demands:
- Work environment- work environment is occasionally hectic with occasional periods of high stress. Occasional to moderate short deadlines and coverage require flexibility in schedule.
- Physical demands- must be physically able to occasionally lift a minimum of 25 pounds and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Emergency behavioral intervention in accordance with behavioral intervention techniques and individual behavioral plans. Driving a passenger van and vehicle may be required.
Travel:
Must have access to personal vehicle during all scheduled work hours. Frequently will travel with supported individuals utilizing personal vehicle as well Community Choices' vehicles. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards.
