Job/Employment Coach for Adults Needing Support
FOCUS POINT BEHAVIORAL HEALTH is seeking a compassionate and dedicated Job Coach to join our team, supporting adults with learning disabilities in their journey towards successful integration into the workforce. If you are passionate about making a positive impact on individuals' lives and have a strong understanding of the challenges faced by the clients we serve, want to hear from you. We are looking for Job Coaches in Baltimore City, Somerset, Wicomico, Dorchester and Talbot Counties.
Position: Job Coach - Responsibilities:
- Network with local businesses/ employers to develop employment opportunities for individuals that require Supported Employment.
- Provide vocational counseling and job coaching to individuals.
- Complete job/task analysis; including developing and implementing job training plans.
- Monitor job placements with a goal 90-180-day retention.
- Visit job and internship sites and meet with individuals on-site, as scheduled, including evenings and weekends.
- Provide intensive skills training and employment adjustment counseling at the job site.
- Monitor individuals work adjustment through direct observation and through meetings with employers and supervisors.
- Meet with individuals for follow-up counseling at the agency site.
- Develop, monitor, implement and document employment and rehabilitation treatment planning goals.
- Develop and prepare written assessment of individuals adjustment to work and file them in case Electronic Health Record (EHR).
- Address problems and implement strategies as outlined in the individual treatment plans of program participants.
- Document contacts, problem areas addressed, and implemented strategies with program participants.
- Assist in securing community and generic resources for program participants.
- Document interactions, progress etc. in the individual’s Electronic Health Record (EHR).
- Attends case conferences or interdisciplinary team meetings and training, and development workshops.
- Job coaching with individuals to provide support in maintaining employment.
- Must have driver license with 2 points or less and a reliable vehicle.
- Full coverage vehicle insurance.
Requirements and Qualifications:
- Possess an associate degree in human services or equivalent education/experience. - OR - 5-year minimum experience.
- Excellent time management skills and ability to multitask and prioritize work.
- Attention to detail and problem-solving skills.
- Excellent written and verbal communication skills.
- Strong organizational and planning skills.
- Proficiency in MS Office.
- Minimum two (2+) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders.
