About Us:
Home Based Primary Care consists of the health care services provided to Veterans in their home. This program is for Veterans who need team based in-home support for ongoing diseases and illnesses that affect their health and daily activities.
Our Home-Based Primary Care program is dedicated to providing comprehensive medical and psychological care to veterans in their homes. With a focus on promoting independence, dignity, and quality of life, our interdisciplinary team delivers patient-centered care that addresses the unique needs of each individual.
Key Responsibilities:
- Conduct in-home assessments to evaluate psychological and emotional well-being, cognitive functioning, and support needs of veterans.
- Provide individual and family counseling to address a range of psychological and interpersonal issues, including coping with cognitive changes and managing chronic health conditions.
- Collaborate with other members of the interdisciplinary team, including physicians, nurse practitioners, social workers, and occupational therapists, to develop and implement integrated care plans.
- Offer education and support to family members and caregivers, including connecting them with community resources and services that may be beneficial for the veteran's care and well-being.
- Participate in team meetings, case conferences, and care coordination activities to ensure continuity of care and effective communication among team members.
- Document assessments, interventions, and recommendations in electronic health records in a timely and accurate manner.
Qualifications:
- Doctoral degree in Clinical Psychology or Counseling Psychology from an accredited institution.
- Current and active license as a psychologist in any U.S. State or Territory
- Experience working with older adults, particularly those with cognitive changes and complex medical needs, is preferred.
- Comfortable with and capable of working in alternate worksites, including conducting in-home visits as part of a multidisciplinary team.
- Strong interpersonal and communication skills, with the ability to establish rapport and build therapeutic relationships with veterans and their families.
- Commitment to cultural competence and sensitivity in working with diverse populations, including veterans from different backgrounds and military experiences.
