Hope Health is a faith-based, Federally Qualified Healthcare Center providing quality healthcare to the Ellis county community.
We offer Adult Medicine, Women's Healthcare, Pediatrics, Behavioral Health, Psychiatry and Dental services.
We are looking for a LCSW to join our behavioral health patient care team in our brand new clinic in Waxahachie. The ideal candidate will value a team based approach to patient care, be able to work independently, and have excellent communication and interpersonal skills.
General Summary
Provides direct patient behavioral health care to those who have been referred by the providers of Hope Health. Provides counseling and casework to patients with behavioral health issues. Provides crisis intervention as needed and works as a member of a primary care team.
DUTIES & RESPONSIBILITIES
- Assess, diagnose, and provide support to adults and children with behavioral health needs
- Deliver evidence-based psychotherapy, including motivational interviewing, cognitive behavioral therapy, and play therapy
- Conduct crisis assessments and emergency case management assessments as needed
- Conduct groups and/or parenting classes as requested
- Maintain patient records according to standards
- Implement a collaborative model for physical and behavioral health integration, which includes addressing substance use, weight management, chronic pain, and smoking cessation needs
- Attend provider meetings and all other meetings assigned by CEO of Hope Health
- Maintain a network of behavioral health resources in the community
- Work effectively within the behavioral health team, which may include providing supervision to graduate interns
- Contribute to the organization’s inclusive, supportive, trauma-informed culture and mission
- Utilize the allotted professional development budget to engage in ongoing learning of evidence-based interventions
- Maintain license in good standing according to the rules and regulations of the Texas State Board
- Follow the ethics and professional standards of the State Licensing Board
- Any and all other duties as assigned by CEO or COO of Hope Health
KNOWLEDGE, SKILLS & ABILITIES:
- Work in a self-motivated, independent way
- Ability to apply your knowledge and research to clinical problems
- Excellent communication and interpersonal skills in order to serve patients in distress
- Ability to collaborate with colleagues from other disciplines as needed
- Ability to use Electronic Health Records
EDUCATION & EXPERIENCE
- Master’s Degree, required
- Licensed Clinical Social Worker or Licensed Professional Counselor, required
- License must be in good standing and allow practitioner to provide services in Texas
- Bilingual Spanish/English, preferred
Job Type: Full-time
Expected hours: 40 per week
