Purpose:
The Lead Intake Coordinator is a skilled intake coordinator whose job is to maintain and manage the incoming patient referrals and supervise the Intake Staff. This person will also assist with other duties relating to compliance in the department as well as other office functions.
Job Description:
Presents a positive, professional image of the organization on the phone and in all interactions with clients, patient/family members, staff and others
Manages and supervises all Intake Department staff and functions to ensure a high level of communication, accuracy and completion of all intake requirements as well as day to day oversight and support
Ensuring that all new referrals are processed immediately (within 15 minutes of receipt during business hours) to ensure compliance, follow up and track:
Ensure all insurance information is captured accurately including primary, secondary insurance, and information on room and board payers as needed
Confirm receipt and accurate processing of all consents, correct orders, patient Information sheets, Oral CTIs, business office approvals, clinicals and any other relevant information that is required
Track and document in EMR on referral status including NTUCs and NTUC reasons, patients who are skilled and date coming off of skilled care, and all information required for any transfers GHS
Ensure documentation is in the EMR regarding any delay admitting or assessing the patient i.e. if the family is asking us to wait a week
Any other documents needed for billing
Work with Community Liaisons and Teams to ensure smooth transition, questions are answered and all information and documents are in progress as we work towards timely admission
Alert appropriate team, managers and community liaisons and other appropriate parties when a referral is received
Oversee and manage the EOD Reports that are shared to teams ensuring all reports are updated daily and sent out to appropriate teams
Manage and share the Audit report with direct reports and supervisor to ensure all required documentation is in the patient chart ahead of Compliance and in support of MRC reviews
Qualifications:
- High school graduate or equivalent
- Three years relevant prior work experience
- Excellent communication & organizational skills
- Previous management skills preferred
- Computer literate in Word, Excel and PowerPoint
- Able to handle multiple tasks
- Good typing skills
- Excellent telephone skills and professionalism
