Under the general supervision of the Laboratory Support Services Manager, performs all of the duties and responsibilities of a Phlebotomist. In addition, coordinates daily phlebotomy assignments and staffing, assists in orientation, training and competency assessment of staff, participates in Quality and Safety programs and assists with Supply Inventory and Ordering. Maintain the professional standards as outlined in the phlebotomy career ladder and have working knowledge of registration duties required of outpatient phlebotomy stations.

- 1. Collects and delivers blood samples on patients in a safe and efficient manner, following department standards to support quality care.1 .1 Follows universal precaution standards for each blood collection, 100% of the time.1.2 Responsible for labeling all specimens legibly and accurately, without error at the time of draw; as required by hospital and departmental standards to ensure accurate result reporting.1.3 Communicates in a professional and concise manner, procedure to be administered to the patient for the purpose of quality patient care.1.4 Collects Blood Specimens appropriately for all patient populations, following established departmental standards.1.5 Packages and/or transports specimens, following Laboratory requirements to ensure timely arrival of specimens in the main Laboratory.1.6 Performs specialized testing as required; including but not limited to: Blood Culture Collection, and Glucose Tolerance Testing.1.7 Reviews requisitions for completeness of information to include diagnosis, physician signature and special instructions. If incomplete, follows through by contacting physician office for correct information prior to blood collection.
- 2. Participates in daily upkeep of blood draw area and Supply Inventory.2.1 Maintains cleanliness and neatness of blood draw room and/or mobile phlebotomy cart and tray.2.2 Documents cleaning logs as required by Department standards.2.3 Through a team approach, maintains neatness of working area.2.4 Performs cleaning of equipment as needed. Updates maintenance logs or records as required.2.5 Stocks supplies for inpatient or outpatient phlebotomy daily and as needed to maintain patient care.2.6 Monitors supply inventory to insure adequate volume of supplies and prepares supply orders as needed. 2.7 Uses YNHH Inventory System to place supply orders.
- 3. Assists in Orientation, Training and Competency Evaluation of employees and Phlebotomy Students 3 .1 Assists in Orientation and training of new personnel and students.3 .2 Communicates training procedures in a positive and effective manner.3.3 Completes documentation regarding training process, such as training checklists.3.4 Observes phlebotomy staff performing venipuncture and related computer tasks to assist Manager with validation of venipuncture and computer skills.
- 4. Performs all functions relating to Laboratory Medicine patient intake information.4.1. Using Hospital standards, chooses existing or creates new medical record numbers in t he YNHH HIS and creates a complete registration.4 .2 Obtains necessary signatures, scans all required patient information including photo ID andinsurance cards.4 .3 If required, produces Laboratory Medicine ABN, provides financial explanation regarding the form to the patient and scans signed form into YNHH HIS.
- 5. Consistently enters lab test orders into the HIS or LIS accurately and efficiently. Uses hand -held scanner for Inpatient Identification and collection label generation.5 .1 For Inpatient collection, consistently demonstrates proficiency with hand-held device used to identify patient, print labels and collect specimens.5 .2 For outpatients: follows established procedures when transcribing patient lab orders and/ or releasing orders from the HIS for specimen collection. 5 .3 Processes patient lab orders efficiently to insure minimal patient wait times. 5.4 Consistently maintains a low ratio of errors to patient draws.
- 6. Transports inpatient specimens; processes outpatient specimens for courier transport in order to facilitate timely specimen testing.6 .1 Follows established specimen handling guidelines and precautions.6.2 Demonstrates ability to use Pneumatic Tube system, including proper packaging of specimens for transport in system carriers.6.3 Prioritizes STAT sample requests, insuring timely delivery to the Laboratory.6.4 When assigned to outpatient area, processes specimens, including centrifugation and storage of specimens if needed, according to established procedures.6 .5 When assigned to outpatient area, prepares specimen tracking manifest for each shipment ofspecimens to Central Processing.6 .6 When assigned to outpatient area, consistently packages specimens for timely transport to the main laboratory.6.7 Responsible for maintaining specimen integrity from the point of obtaining the specimen, through transport to the Laboratory.
- 7. Adheres to regulatory standards for Laboratory Testing7.1 Consistently captures ABN information for Medicare Patients when required for Outpatient services.7.2 Maintains technical competency for phlebotomy and related tasks.7.3 Signs off on Departmental Procedures and Updates in a timely manner as required.7.4 Follows established procedures for Self-Pay test requests for outpatients, including obtaining Waiver Forms when needed for outpatients.
- 8. Coordinates Daily Phlebotomy assignments and staffing.8.1 Completes daily phlebotomy assignments for each shift; including coverage for meals and breaks. 8.2 Monitors phlebotomy work queues on a regular basis through the work shift; adjusts work assignments as needed to maintain timely response for phlebotomy requests.8.3 Carries "Charge Phlebotomist pager and responds to all pages in a timely manner.8.4 Assigns phlebotomist to Grimes for scheduled morning rounds and timed/stat requests outside of morning rounds.8.5 Assists Manager with Phlebotomy scheduling; follows established guidelines. Replaces staff for planned and unscheduled vacancies on all shifts. 8.6 Insures that oncoming shift(s) are adequately staffed.8.7 Facilitates the shift to shift hand-off communications for phlebotomy
- 9. Participates in Laboratory and Hospital Safety and Quality Programs.9.1 Represents the Laboratory on at least one Hospital or Laboratory Safety or Quality Committee.9.2 Participates in Laboratory Morning Safety Huddle as assigned.9.3 Obtains feedback from patients regarding venipuncture quality and patient satisfaction.9.4 Assists manger with reports or data collection as needed for Quality and/or Safety.
EDUCATION
High School Diploma. Completion of an approved Phlebotomy Certification Course or equivalent work experience with on the job training.
EXPERIENCE
Three years of full time experience as a Phlebotomist or equivalent part time at YNHHS as a Phlebotomist.
LICENSURE
National Phlebotomy Certification preferred.
SPECIAL SKILLS
Ability to prioritize changing workload demands. Ability to problem solve; ability to recognize potential problems/issues in the day to day work.
PHYSICAL DEMAND
Ability to do extended amounts of walking and bending over patient beds for prolonged period of time.
