Shelter House was established in 1981 as a grassroots responder to the homelessness crisis in Fairfax County.  Since the organization’s inception, we have adapted and grown to meet community needs, all while working in partnership with government and private partners and community members. About the Role:

The Prevention and Diversion Case Manager plays a vital role in supporting people in the community who are at imminent risk of homelessness. This position involves providing direct, time-limited services to participants, including comprehensive assessments of their needs and connecting them with necessary resources such as legal assistance, housing options, and mental health support. The role emphasizes advocacy to ensure participants receive the full spectrum of services required to achieve housing stability. In addition, the Prevention and Diversion Case Manager implements diversion strategies to prevent participants from re-entering crises or the justice system unnecessarily, by identifying alternative support systems and preventive measures. The position requires a client-centered approach that respects self-determination, involves them in decision-making, and empowers them to achieve their goals. Effective collaboration with community partners and service providers is essential for providing holistic support and addressing any gaps in care. The role also involves maintaining accurate documentation and preparing reports to ensure compliance with organizational policies and funding requirements

Responsibilities:

Participant-Centered Support

• Coordinate and manage all prevention and diversion referrals to ensure timely and accurate service delivery.
• Assess and guide households in exploring alternative stable housing options to divert them from emergency shelter.
• Review and respond to referrals from households experiencing or at risk of homelessness.
• Develop, maintain, and share a comprehensive resource library and related communication materials.
• Promote program awareness and build trust with community partners to enhance engagement.
• Provide case management, linking participants to resources such as shelter, permanent housing, health/mental health services, public benefits, and employment opportunities.
• Conduct assessments and make referrals to support participants' goals and needs.
• Create individualized, housing-first service plans and assist participants through the eviction process, including:
• Housing location services, referrals, and advocacy
• Facilitating housing-related tasks such as document collection
• Assisting with rental assistance packages from various funding sources, properly documenting fund distribution, and leading the recertification process as needed.
• Address safety concerns quickly and compassionately.
• Complete and submit all required incident documentation before the end of each shift.
• Assist participants in securing basic needs such as clothing, employment, and childcare.
• Empower participants to successfully achieve their identified goals.
• Provide accessible services to participants, including conducting field visits.

Collaboration & Administrative Support

• Collaborate as part of a multidisciplinary team to ensure timely service delivery.
• Ensure all relevant participant information is promptly shared with the appropriate staff.
• Monitor participant progress through regular contact and communication with external and internal service providers, conducting assessments, and making necessary referrals.
• Manage tasks and maintain up-to-date, complete participant charts, including scanning documentation as needed.
• Ensure accurate and timely documentation of participant interactions within the HMIS database.
• Perform monthly data clean-up to maintain HMIS accuracy.
• Participate in weekly staff meetings and training sessions as directed by the supervisor.
• Proactively communicate caseload coverage and provide updates on outstanding tasks when absent from work.

Requirements

About You:

Required: 

·        A bachelor's degree in human services/ related field or commensurate experience 

·        2+ years of street outreach/case management/ direct services/other related experience 

·        Strong written and oral communication skills 

·        Ability to prioritize competing priorities and make sound judgements 

·        Ability to complete tasks while navigating frequent interruptions 

·        Ability to deliver creative, resourceful solutions to unique challenges 

·        Strong organizational and time management skills 

·        Valid Driver’s License and reliable transportation  

·        Ability to Drive a 15-passenger van 

·        Willingness to work evenings and weekends when needed 

Physical Requirements:

●       Annual TB Test is required

●       Ability to sit or stand for long periods

●       Ability to lift items weighing 10-20 pounds
