Key Responsibilities:

• Patient-Centered Care: Provide comprehensive psychosocial assessments, interventions, and support to patients and families throughout their healthcare journey.
• Care Coordination: Facilitate the smooth transition of care along the continuum, ensuring appropriate utilization of resources and access to necessary services.
• Discharge Planning: Assess and address patients' needs for safe and timely discharge, working to prevent avoidable readmissions.
• Advocacy and Support: Advocate for patients' rights and well-being, connecting them with community resources and support systems.
• Collaboration: Work collaboratively with a multidisciplinary team of healthcare professionals, including physicians, nurses, and other social workers.
• Documentation: Maintain accurate and timely documentation in the Case Management documentation system, including psychosocial assessments, interventions, and progress notes.
• Education and Training: Precept new staff members and serve as a resource for all staff on psychosocial issues and care coordination.
• Quality Improvement: Actively participate in departmental Quality Improvement initiatives, contributing to the continuous improvement of patient care processes.

Qualifications & Experience:

• Required:
  • Master's Degree in Social Work (MSW)
  • Current and active California Licensed Clinical Social Worker (LCSW) license
  • Demonstrated organizational, verbal, and written communication skills
  • Ability to lead and coordinate activities in a fast-paced environment
  • Critical thinking and problem-solving skills
  • Computer literacy
• Preferred:
  • Accredited Case Manager (ACM) certification
  • Minimum of two years of acute hospital experience
