About You
Experience to drive change
- Social Work license required in the State of Indiana. Multi state licenses a plus.
- 3+ years of experience in a healthcare setting preferred
- A bachelor’s degree in social work required
- Comfort with technology including the Microsoft suite of products
- Prior experience using electronic health record a plus
- Social work is incredibly dynamic and requires a solid foundation in social work principles, a keen awareness of community resources, and the ability to navigate complex emotional and social landscapes with empathy and professionalism.

- Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.

About the Job
A Licensed Clinical Social Worker plays a fundamental role in the field of human services, focusing on improving the well-being of individuals, families, and communities.
· Serving as a compassionate advocate and resource facilitator, they work directly with clients to identify and address various social issues, such as poverty, abuse, addiction, and mental health challenges. Assessing for SDOH gaps and understanding how they affect our patients is essential to a wholistic treatment approach to health and wellbeing.
· Conduct assessments to understand the unique needs and circumstances of their clients, providing essential support through counseling, crisis intervention, and connecting them with community resources and services.
· Collaborating with other healthcare providers, staff members, and social service organizations to develop comprehensive case plans and actions that promote client stability and self-sufficiency.
· Engage in outreach efforts to coordinate services and resources to meet the needs of vulnerable populations.
· With a commitment to ethical practice and cultural competence, Social Worker practitioners play a crucial part in fostering social justice and enhancing the quality of life for those we serve.

The working environment and physical requirements of the job include:
This position requires both in-home and office-based work. The job requires frequent travel for home visits and travel to physician offices, hospitals, sub-acute facilities, community partners and non-home-based market offices in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.
In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
