Primary Responsibilities

• Provide compassionate and personalised clinical social work services to patients in a post-acute setting
• Collaborate effectively with an interdisciplinary healthcare team to enhance patient outcomes
• Utilise Electronic Medical Records (EMR) accurately and efficiently
• Maintain empathetic and professional communication with patients and their families

Required Qualifications and Skills

• Master of Social Work (MSW) degree from an accredited institution
• Valid LCSW license in good standing in Virginia
• Proficient in the use of EMR systems
• Exemplify compassion, intelligence, motivation, and teamwork skills
