Loyal Source Government Services is looking for a Licensed Clinical Social Worker – Mental Health work at an Outpatient Clinic in Auburn, NY.
Job Summary: Licensed Clinical Social Worker – Mental Health - to work at an outpatient clinic
Key Responsibilities: Responsible for collaboration with PACT team for the provision of mental health services to enrolled and unassigned patients presenting for care per VHA Handbook 1160.01 and the requirements of this contract. Covered services would include, but are not be limited to, conducting bio-psychosocial assessments to determine needs of Veterans, Veteran’s family members and caregivers. Provide a full range of clinical social work services within established standards of social work practice which includes treatment planning, individual, family and group counseling, VA and community based resource referrals, case management and other services as appropriate
Requirements:
Qualifications: Social Workers providing services under this contract must have a Master’s degree in Social Work (MSW) from a school accredited by Council on Social Work Education (CSWE). Current, full, active, and unrestricted license in the state in which the Outpatient Site of Care (i.e. CBOC) is located
