- Department: Primary Care Clinic
- Schedule: FT, Monday-Friday, 8am-5pm
- Location: Sand Springs, OK
Provides behavioral health counseling services.
- Conduct patient assessments and establish therapy treatment goals.
- Administer individual and group therapy.
- Maintain thorough and confidential records of observations.
- Evaluate progress and effectiveness of therapy interventions.
- Provide patient with information regarding resources and therapeutic activities in the community
- Work directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge.
Licensure / Certification / Registration:
One or more of the following:
- Counselor credentialed from the Oklahoma State Board of Behavioral Health.
- Social Worker specializing in Clinical Practice credentialed from the Oklahoma State Board of
Licensed Social Workers. - Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) required.
Preferred Credential(s):
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Clinical Counselor preferred
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
-LPC, LCSW or LADC-MH licensed in good standing in the state of Oklahoma
-Ability to work in a collaborative care model with medical providers
-Strong and timely written and verbal communication
-Ability to manage own caseload from initiation to termination
-Primary location at Sand Springs with secondary location at another Tulsa area clinic
