Licensed Director of Social Work/ FutureCare Health and Management Corporation - City of Baltimore, Mayor's Office of Employment Development

Responsibilities:
• Coordinate and supervise medically related social services provided by
the facility's staff to assist residents in maintaining or improving their
ability to manage their everyday physical, mental, and psychosocial
needs
• Supervise programs designated to the Social Work Department
• Participate in daily meetings with the interdisciplinary team
• Assist in meeting the psychosocial needs of residents, their families,
and significant others as necessary in coping with the effects of illness,
disability, treatments and stay in the facility
• Address advance directives and assists in the determination of the
appropriate medical decision maker
• Provide individual, family and group services designed to maximize
psychosocial well-being


Qualifications:
• Must have either a bachelor's degree (required) and/or master's
degree (highly preferred) in Social Work
• Must possess a current valid Maryland Social Work license
• Must have prior experience in a skilled nursing and Long-Term Care
environment
• Management experience in an interdisciplinary setting is preferred
• The ability to lead and develop the department and its staff member
