A Prestigious hospital located in New York, NY has an immediate opening for a Licensed Master Social Worker with a background in medical Social Work
Position Type: Contract
Schedule: 09:00am-05:00pm (includes a 60 min break)
Location: New York, NY
Responsibilities:
Provides social work services to patients and families to assist them in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning.
Collaborates with Hospital, New York-Presbyterian System, and community-based personnel to ensure efficient service delivery and a high level of customer satisfaction.
Requirements:
LMSW in NY
Masters in SW (R)
2+ years in acute care medical social work experience, including hospital discharge planning
Inpatient discharge planning hospital experience.
EPIC experience
Experience with Adolescent and pediatrics population preferred
Versed in conducting psycho-social and mental health assessments with emphasis on family systems and trauma informed care-able to provide referrals to community-based resources to assist with any concrete needs that may arise -able to work in a team setting
