We are looking for an LVN to join our team in Davis, CA! This is a full-time position offering full benefits, as well as competitive pay. Your shift would be 7 days on and 7 days off, so you have plenty of time between shifts to have a great work/life balance.

- Triages phone calls from patients/families.
- Makes home visits as staffing issues dictate. Works cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.
- Reports DPCS/Clinical Manager and day shift nurses with patient updates from night shift with daily reports and EMR charting.
