Responsibilities of the Licensed Practical Nurse (LPN):
- Consult and coordinate with interdisciplinary care team, which includes medical providers and other healthcare professionals to assess, plan, implement and evaluate individualized patient and resident plans of care.
- Participate in completing baseline and periodic assessments of individualized care needs to ensure the most appropriate care needs are met for each patient and resident.
- Participate in resident care, to include accurately and safely administering medications that are ordered by our medical providers, and monitoring to ensure efficacy of medications provided.
- Provide nursing services to short term patients and long-term residents within scope of practice, organizational policies, and professional standards of care.
Requirements of the Licensed Practical Nurse (LPN):
- Current license from state
- Current/active CPR certification, preferred.
- Effective verbal and written English communication skills.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
- Able to stand and/or walk for more than four (4) hours per day.
- Able to bend and/or stoop for more than one (1) hour at a time.
