Licensed Practical Nurse - Hannah Duston Healthcare Center
Full-Time, 40 hours/week - all shifts
Part time 24-32 hours/week- all shifts
Baylor 7a-7p, 7p-7a every weekend. Work 24 hours, get paid for 32
Base Pay: $33.87-41.00
Differentials offered for evening, night and weekend shifts
Essential Duties: To provide exceptional care to all residents including but not limited to:
- Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary
- Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary
- Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action
- Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily
- Performs comprehensive assessments of each resident on unit
- Develops care plan based on each resident's assessment
- Obtains physician orders and any revisions for all treatment and medications needed
- Assesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of condition
- Renders immediate assistance to any resident in need
- Supervise and evaluate all direct resident care and initiates corrective action as necessary
- Provides clinical supervision of nursing assistants providing direct patient care
- Assumes responsibility for compliance with federal, state and local regulations within the assigned unit
- Counsels nursing assistants and communicates clearly with Clinical Nurse Managers and/or Director of Nursing if issues arise
- Collaborates with interdisciplinary team to assure that high quality resident care is maintained
- Prepares nurse assistant assignments and explains all pertinent resident care
- Supervises distribution of nourishments
- Supervises the removal of all postural supports
- Orders, receives and documents medication according to facility procedure
- Supervises response to residents calls for assistance and responds to call lights as necessary
- Insures that all physician renewal orders and physician documentation is timely and appropriate
Prerequisites:
- Graduate of an accredited Licensed Practical Nurse school of nursing
- Current and active LPN license in the state of Massachusetts
- Current certification in Basic Cardiac Life Support in accordance with AHS or ARC standards
- Knowledge of Geriatric Nursing required
