Pay Range: $25.00 - $35.00/hr
JOB SUMMARY
The LPN is responsible for following the patient’s plan of
care and providing treatment under the supervisor of the RN.
DUTIES AND RESPSONSIBILITY
- Implement plan of care initiated by the registered nurse
- Provide accurate and timely documentation consistent with the plan of care
- Collect assessment data and performs observation to report to and collaborate with RN(s) and physician(s)
- Participate in coordination of home health services with interdisciplinary team
- Use equipment and supplies effectively and efficiently
- Participate in personal and professional growth and development
JOB REQUIREMENTS
- Valid Colorado Licensed Practical Nurse License
- Maintain a current CPR certification
- 2+ years of experience working in direct patient care
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
