Halifax Health Hospice is seeking a Licensed Practical Nurse (LPN).
Summary:
The Licensed Practical Nurse will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse. Job responsibilities include carrying out selected procedures, administration of medications, and treatments.
Job Qualifications:- Currently licensed or eligible for LPN licensure in State of Florida.
- Graduation from an approved School of Nursing in a course of training for Licensed Practical Nurses.
- Must have one year's experience in Hospice, Med-surg or other acute care area.
- Valid driving license and good driving record are required for this position.
- Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians and ancillary department personnel.
- On-Call responsibilities required.
- Certification from a 30 hour IV course preferred.
- Additional certification provided through the facility for IV responsibilities after certification recieved.
- Provides compassionate care to the terminally ill and their families.
- Perform and document complete assessments.
- Reports any change to supervising RN.
- Provides patient and family education.
- Reinforces the implications of the diagnosis/prognosis and the nature of treatment based on information from primary nurse and/or physician.
- Works in collaboration with an Interdisciplinary Team to develop a Plan of Care which reflect pt./family centered goals.
- Uses critical thinking skills.
- Functions as a representative of HOVF and utilizes effective communication methods and skills.
- Participates in quality improvement activities.
- Maintains professional and positive working relationships.
- Promotes a holistic approach to the care of the terminally ill
