• Provides assistance to referral sources electronically or onsite to review patient information to obtain demographic, insurance and relevant clinical information.
• Meets with patient/family to settle home care needs resources to assist with safe discharge to home setting.
• Acquires, enters, and updates consents, Advance Directive Information, Patient Payment responsibility and Medicare Secondary Payer information, insurance authorization and pre-authorization
• Receives, documents and communicates physician orders for HomeHealth Supports the intake process by interacting with discharge planners, case managers and follows patients throughout patient care transitions.
• Coordinates with appropriate departments to schedule the start of care to meet patient needs Serve as subject matter authority to referral sources on HomeHealth services including virtual visits, private pay and adult day program

ChristianaCare HomeHealth is seeking a Full Time LPN Liaison. As a  Nurse Liaison for ChristianaCare HomeHealth you will work in the Southern Delaware area, specifically the greater Milford area, identifying home care needs of patients to ensure a smooth transition from acute care and post acute care settings.  The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients.

Requirements:

• LPN licensed in the State of Delaware, or an appropriate State based upon work to be conducted within the State of Delaware.
• 1 year experience in home health, hospice or care coordination preferred
• Knowledge of ChristianaCare HomeHealth service lines.
