Covering Areas: (Logan Team: Logan Chelsea, East Boston, Revere, Winthrop)

1 year Certified Home Care experience required. 

Weekend rotations are once every 6 weeks. Ability to travel to cases as assigned. 

Job Summary: 

Provide quality nursing care to patients in various settings in the community. 

In collaboration with the RN case manager to provide skilled nursing visits.

Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care.
