Job Description Summary
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Essential Job Functions/Responsibilities
- Provides direct patient care as defined in Connecticut State Nurse Practice Act. The LPN can perform, but is not limited to, IV care and maintenance, injections, dressing changes, ostomy care, enteral feedings, urological catheter changes, medication prepours and patient education, under the direction of the registered nurse. Information gathered from home visits is communicated to the primary care nurse and/or supervisor. The LPN is restricted from performing initial patient assessments, OASIS assessments (start of care, resumption of care, follow-up, SCIC or discharge), and home health aide orients or supervision in the plan of care.
- Implements plan of care initiated by the registered nurse, as delegated.
- Receives and provides updates and reports daily on each patient care is delegated for. Ensures the primary care nurse is informed of observations, effect of education and items requiring follow up.
- Reports to the primary care nurse or clinical supervisor any change in patient condition, s/sx of deterioration or complication.
- Provides accurate and timely documentation consistent with the plan of care per agency Compliance Plan to prevent fraud and abuse and agency policy and procedure.
- Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care under direction of the registered nurse.
- Participates in coordination of home health services (HHA, OT, PT, MSW, ST, Dietician) with members of the interdisciplinary team.
- Uses equipment and supplies effectively and efficiently and helps arrange for supply and DME acquisition, as needed.
- Participates in agency weekend and holiday program.
- Participates in personal and professional growth and development.
- Personally implements, promotes and enforces the Stratford VNA Corporate Values System.
- Conducts all communication and interaction in a positive, professional and customer service-focused manner, both inside the agency, as well as with outside entities.
- Is flexible and cooperative in fulfilling role obligation and accepts cases as assigned.
- Attends all nursing inservices and meetings as scheduled, in person or with supervisory approval, arranges for make-up sessions or videos.
- Demonstrates competent clinical skills upon completion of orientation and at least annually thereafter as indicated on the nursing competency checklist.
- Performs other duties as assigned by the Registered Nurse.

Position Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Has minimum one year nursing experience. Community/home health or medical/surgical experience is preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the State.
- Possesses and maintains current CPR certification.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
