We’re currently looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join us in Tucson, AZ, to provide exceptional care and support to our patients and their families.

Key Responsibilities:

• Identify patient and family needs and provide supportive care in line with physician orders and interdisciplinary care plans
• Work closely with the Nurse Case Manager to assess and manage all phases of the nursing process
• Communicate effectively with physicians, team members, and other agencies to ensure patients receive the best possible care
• Maintain ongoing communication with the Nurse Case Manager, Program Director, and Medical Director
• Collect and report data on physical, emotional, social, and spiritual factors affecting patient and family well-being

What We’re Looking For:

• Current LPN/LVN license in Arizona
• Strong verbal and written communication skills and excellent organization
• Licensed driver with an insured vehicle in good condition
• Current BLS certification
• Compassionate nursing skills with the ability to support families emotionally and respect individual lifestyles
• Fingerprint Clearance Card (or willing to obtain)
• Home Health experience a plus
• Bilingual in English/Spanish is strongly preferred!
