An amazing new opportunity has come up in our Residential Program! We are looking for a residential Licensed Practical Nurse (LPN) to join us Full-Time or Part Time on our residential team, specifically in Topsfield.
Job Summary: Provides direct care to individuals living in a residential setting. Follows outlined plan of care, identifies health-related needs, monitors medical status and reports changes to the on-call RN, and collaborates with residential staff to support the individuals in their community residence.
Major Responsibilities:
- Health Monitoring & Care:
- Conducts regular health assessments (on shift), including vital signs and symptom checks, to monitor individual health & determine nursing needs. Reports to and seeks direction from the RN Case Manager or on-call RN as needed.
- Medication Management –Administers medications and treatments as prescribed and per MAP regulations. Manages medication storage and documentation in compliance with MAP regulatory standards and NeArc organizational policies.
- Demonstrates competence in trach care, vent care & related emergency procedures.
- New Grads must successfully complete in-house training and demonstrate competence to the program RN Case Manager prior to providing care to individuals that utilize a trach &/or vent.
- Care Plan Implementation:
- Understands clinical and social needs of individuals receiving services.
- Assists in the development and implementation of individual specialized care plans.
- Completes and maintains written records as assigned by Supervisor. Prepares and submits all paperwork in a timely manner. Including but not limited to daily data collection, incident and other event-specific reports, progress notes, and data summaries.
- Utilizes nursing knowledge and skills to safely implement basic preventative care and therapeutic and rehabilitative interventions.
- Medical Coordination & Communication:
- Assists program director with scheduling and preparing individuals & documentation for medical appointments, accompanies as necessary, and ensures that medical orders are implemented per MAP and followed up.
- Communicates any change in individual’s status, orders or plan of care to the program RN Case Manager, Program Director, on-call RN, and/or designee.
- Presents information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and/or email correspondences.
- Provides change of shift report to incoming staff for individuals under your care.
- Follows HIPPA and security regulations and/or guidelines to maintain confidentiality at all times.
- Participates in supervision, general staff meetings, and trainings as scheduled.
- Compliance:
- Maintains universal precautions at all times and implement all NeArc and Residential Division Standards related to Health and Safety.
- Maintains a license in good standing and certifications as required by the program, the funding source, and OSHA including but not limited to provider-level CPR, Universal Precautions, and Human Rights.
- Demonstrates and implements knowledge of NeArc Policies and Procedures, including Standards of Conduct and Residential Division Standards.
- Appropriately manages agency funds, obtains receipts for all purchases and adhere to NeArc policy and procedures.
Qualifications:
- Licensed LPN in Massachusetts with minimum of 1 year of experience
- Experience working with individuals with intellectual or developmental disabilities preferred
- Ability to develop and maintain a respectful, caring, tolerant, ethical and empowering relationship with individuals with intellectual and physical disabilities
- Must maintain CPR certification and complete required programmatic trainings
- Must have a valid driver’s license and reliable transportation
Job Type: Fulltime, Part Time, or Per Diem
Shifts available: First 7:00am -3:00pm, Second 3:00pm-11:00pm, Third 11:00pm-7:00am
Pay Rate: $38/hr
Work Environment: Works in the community in a home setting with either multiple residents or someone who lives independently.
