Licensed Practical Nurse - McKnight Place Extended Care

McKnight Place Extended Care, an exclusive Long Term Care Facility, is currently seeking a LPN with Medicare experience to fill a Full-Time position on our  Evening/Night Shift.  We are looking for a caring, dependable, professional individual who desires to give excellent care to our Residents. As an LPN, you will provide leadership as well as being part of an outstanding team. Nurse will provide direct nursing care to all residents of McKnight Place Extended Care, in accordance with State and Federal regulations and administrative policies as well as “Best Practice.’

Responsibilities:

· You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

· LPN will recognize and interpret changes in residents’ conditions, notify physician and families of condition change and take appropriate action as directed by the physician.

· LPN will prepare and monitor CNA and CMT assignments and ensure CNAs and CMTs are aware of resident care plans including updates and changes, if applicable

· LPN will establish and carry out procedures for admission, transfer, and discharge of residents to and from Facility.

· Administer nursing care to ill, injured, or disabled patients

· Diagnose and establish patient treatment plans

· Monitor and report changes in patient symptoms or behavior

· Communicate with collaborating physicians or specialists regarding patient care

· Educate patients and their families about health maintenance and disease prevention

· Facilitate referrals to other healthcare professionals and medical facilities

· Maintain accurate patient medical records

· Provide advice and emotional support to patients and their family members

Qualifications:

· Previous experience in nursing or other medical fields

· Familiarity with medical software and equipment

· Ability to build rapport with patients

· Strong problem solving and critical thinking skills

· Ability to thrive in a fast-paced environment
