Licensed Practical Nurse | Youth - TAG MedStaffing

Role Overview:
We are actively seeking a dedicated and compassionate New York State Licensed Practical Nurse to assume the role of Youth Medical Care Manager within our client's team. In this pivotal position, you will oversee the medical care management of youth in residential care and family foster care.

Compensation: $38.00 - $40.00 / HR
Location: Bronx, New York
Schedule: Flexible Shifts

Responsibilities:

• Identify health needs of adolescents and caretakers through direct interactions
• Establish a medical home for adolescents, addressing current and future healthcare needs
• Serve as a liaison between multiple healthcare service providers
• Create a compassionate environment, providing emotional, psychological, and spiritual support
• Foster independence by setting goals and educating clients, families, and caretakers
• Resolve client issues using multidisciplinary team strategies
• Document care services in patient and department records, ensuring regulatory compliance
• Communicate health information to clients and the multidisciplinary team
• Maintain cooperative relationships within the healthcare team
• Contribute to team efforts to achieve results
• Administer medication and monitor youth's response
• Implement Sanctuary Model tools and commitments for a trauma-sensitive culture
• Perform assigned duties and responsibilities

Qualifications:

• Graduated from an accredited practical nursing program
• Proficient in nursing principles and practices
• Collaborative team player with clients, family, and staff
• Excellent verbal and written communication skills
• Current American Heart/American Red Cross CPR Certificate for all age groups
• Accompanied youngsters to various appointments
• Committed to strength-based and trauma-informed care
