Our Registered Dietitian (RD) and DTR is a fundamental role providing nutritional education, counseling and interventions as appropriate, in the management of nutritional status and disease management for patients to their disease states and therapies.
Essential Functions: (RD/DTR)
Provide clinical nutrition and dietary management support consulting services
Develop monthly, quarterly, annual quality assurance reports
Lead nutrition at risk meetings and provide clinical nutrition interventions and care plans
Complete quality assurance audits to ensure regulatory compliance for state/federal survey preparedness
Participate as an essential Interdisciplinary Team Leader within each account
Communicate with facility representatives for coordination of care and services
Complete nutrition assessments and MDS sections
Participate in weekly team calls and complete QA data
MINIMUM QUALIFICATIONS:
Registered Dietitian (RD) as per Commission on Dietetic Registration
Registered Dietitian (RD) State licensure (LDN)
Valid Driver’s License
Computer experience is preferred and PointClickCare experience is preferred, but will train.
