Licensed Social Services Director - Brushy Creek Post Acute

Brushy Creek Post Acute is a gorgeous 144-bed skilled nursing facility conveniently located next to the hospital in Greer, S.C. This unique property has 12 cottages that each hold 12 patients. Each has its own kitchen and there are gardens and patios for residents and employees to enjoy a little fresh air. We offer the following to our Social Services Director

Successful candidates will have the following:

• Licensing in South Carolina
• Experience in LTC/ Rehab setting
• PCC knowledge is a plus
• Compassion for an underserved population
• Ability to problem-solve and communicate effectively

Your day to day:

You'll meet with families and providers to determine the best plan of action for our residents' care. You'll assist with assessments by interviewing residents and their families and see to it that the highest practicable physical, mental and psychosocial wellbeing of each resident is attained or maintained.
