The VNA Health Group is seeking a Hospice Medical Social Worker. The position would be responsible for providing assessment of social and emotional problems that are, or are expected to be an impediment to the effective treatment of the patient’s medical condition or their rate of recovery.

Responsibilities:
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Develops and implements a social work plan of care
- Collaborates with professional staff on psychosocial problems and appropriate interventions
- Provides patients and families with information on community resource options and assists in linking with needed resources
- Adheres to the organization’s policy and procedure on documentation
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
- Masters Degree in Social Work required with current LCSW licensure
- 1+ year of supervised clinical experience; community health experience preferred
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Ability to work with multiethnic and diverse socioeconomic populations
Working Conditions/Physical Demand: Services are provided in the patient’s home in a community or facility setting
