Licensed Social Worker - Madison Health & Rehabilitation

Education Requirements:

Master's degree in Social Work or a Masster's degree in Human Services with a minimum of two years experience in Long Term Care.

Must be a Licensed Clinical Social Worker (LCSW)

The Social Services Director is responsible for planning, developing, and directing the Social Services Department. The SSD will assess the residents cognition, psychosocial well-being, mood, behavior and overall well-being, Assist with admission and discharge planning, care planning, networking, and group meetings. Ensure the residents medical social services needs are met. Be a functional member of the QAPI Committee. Must be available for some evenings and weekends.
