Position Summary
A Licensed Vocational Nurse (LVN), or Licensed Practical Nurse, is responsible for supporting medical professionals to administer patient care. The LVN would be assigned to various assisted living facilities and will work closely with other healthcare professionals and administrative staff members to provide medical treatment to patients with a wide range of injuries or illnesses. LVN may also be responsible for assisting administrative staff in uploading patient files into a digital database or taking phone calls from patients regarding schedule changes.
Essential Duties
• Provide assessments and create care plans for individuals with intellectual disabilities who live in an Adult Residential Facility for Patients with Special Healthcare Needs (ARFPSHN) .
• Responsible for recording and updating patient histories.
• Be able to tell for provision of care.
• Be knowledgeable in signs and symptoms and/or diagnosis.
• Train staff in proper techniques for care of residents.
• Continually assessing resident care needs.
• Take residents on outings with day program staff.
• Give weekly report of all 4 homes.

Qualifications & Education
Position Requirements
• California Nursing License
• California Driver’s license (clear and without restrictions)
• CPR/First Aid Certification
• Current TB test & physical
• Previous experience in the health care field or teaching individuals with disabilities is a plus.

Working Environment and Physical Demands
A typical work location is a home/assisting care facility. Site layouts may vary, and employees are to comply with established safety protocols, including following proper infection prevention and control procedures. Employees are to follow formal instructions on the safe and proper usage of equipment. Potential hazards may include exposure to heavy equipment, chemicals, and potential exposure to communicable diseases, blood-borne pathogens, medicinal preparations, and other conditions common to a community home environment.
Employees will have daily on-going interaction with residents with developmental disabilities who may display aggressive and/or self-injurious behaviors. Employees will have ongoing communication with family members, co-workers, facility staff, and other third parties.

Incumbents must possess and maintain sufficient strength, agility, and endurance to perform the duties outlined in this job description. The employee will perform a variety of physical activities including standing, walking, bending, twisting, reaching at or above shoulder level, gripping, grasping, lifting, carrying, pushing, and pulling. Occasional running, climbing, kneeling, traveling, squatting, sitting, and balancing as required. In some instances, more than one of these activities occur simultaneously and may be rigorous in nature. Employees must maintain the ability to safely operate a motor vehicle.
