Home Healthcare: We provide physician-ordered skilled nursing and health care services to homebound patients recovering from illness or injury. ANX Hospice Care: ANX also provides 24/7 hospice care for individuals with limited life expectancies or for patients no longer willing to seek aggressive medical treatment. Position Summary:

• Must be able to work on call as continuous care nurse 4pm-12 midnight
• Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
• Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
• Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).

Minimum Qualifications:

• Current CA Licensed Vocational Nurse (LVN) license
• Current IV Certification
• Current CPR card for Healthcare Providers
• 1 year of Licensed Vocational Nurse (LVN) experience
• Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
• Demonstrates excellent verbal, written communication, and organization skills.
