Summary
Under the direction and supervision of an MD, NP, and/or RN, educates clients on medications and side effects, TB, HIV and hepatitis and the use of drugs and alcohol. Also, provides case coordination for more complex patients with multiple medications or conditions. The LVN / LPT combines an understanding of the System of Care with critical medical, psychiatric symptom and drug interaction knowledge to support and enhance clinical team efforts in providing service to clients.
Essential Duties/Responsibilities
- Provides case management/linkage services for more complex clients with multiple medications or conditions
- Follows-up with issuing medical doctors orders and pharmacies to ensure prescriptions filled accurately and
promptly - Under direction and supervision of a medical doctor, may dispense and administer medications
- Establishes and maintains working relationships with community resources to assist in case management
- Educates clients on various diseases such as TB, HIV and hepatitis
- Counsels clients on the effect of alcohol and drug
- Teaches parenting skills and child development education
- Functions as a resource to client, family and community resources and allied professionals
- Other duties as assigned or necessary to support the program and/or the company

Given, Does not supervise others, and works under immediate supervision.
Education, Experience, and Licensing Requirements
- Graduate from an accredited vocational nursing school or accredited school for Psychiatric Technicians.
- High school diploma or general education degree (GED) required with computer or business courses.
- Bachelor’s degree preferred but not required.
- Current license to practice in the resident state.
- Proof of: valid CA driver’s license, valid auto insurance coverage as well as proof of education are required.
Knowledge, Skills and Experience
- One year acute care experience and community clinic experience preferred.
- Perinatal and substance abuse experience and education required
- Knowledge and experience working with the severely and persistently mentally ill population.
- Good organizational skills and communication skills, both written and verbal required.
- Intermediate level PC skills required.
- Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must.
- Ability to effectively prioritize and manage time
Physical Requirements
- Office Environment. Requires extensive sitting with periodic standing and walking;
- While performing the duties of this job, the employee is frequently required to stand; walk and sit.
- May be required to occasionally lift and/or move up to 25 pounds.
- Requires significant use of computer, tablet, phone and/or general office equipment.
- Needs adequate visual acuity, ability to grasp and handle objects.
- Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.
- May require off-site travel up to 20% (local) - meeting, county and/or client or program needs.
- Will use personal car or vehicle as required for travel and/or transportation.
Decision Making Responsibilities
- Recognizes any unusual client situations or circumstances and reports them to supervisor immediately.
- Ability to apply common sense in assisting medical doctors.
