Adult Day Program Schedule: 8:00 AM to 3:30 PM, Monday through Friday

Position will be temporarily stationed at our Chatsworth program, with future plans to relocate to Thousand Oaks.

As a Licensed Vocational Nurse (LVN) at Momentum, you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Adult Day Program (ADP). This role offers you the opportunity to work in a small, close-knit community where you can build meaningful relationships with both the people we support and your fellow dedicated team members.

Licensed Vocational Nurse (LVN) Essential Responsibilities: 

• Provide skilled nursing care to individuals with disabilities in an Adult Day Program setting
• Implement and oversee the health care plans for people supported 
• Administer medications and perform medical procedures as needed
• Collaborate with a multi-disciplinary team to ensure the well-being of people supported 
• Maintain accurate medical records and documentation
• Offer support and guidance to Direct Support Professionals (DSP) and other ADP team members 
• Participate in complex case management and engage in continuous learning opportunities

Licensed Vocational Nurse (LVN) Qualifications:

• Valid License Vocational Nurse (LVN) license in the state of California
• CPR certification
• Strong communication and interpersonal skills
• A genuine passion for providing quality care and support to individuals with disabilities
• Ability to work effectively in a team-oriented environment
• Willingness to adapt to the needs of the people supported and facility

Additional Information:

A $2,000 Sign-On Bonus* will be paid in two payments: the first payment after 90 days of employment and the second payment after 6 months of employment.
