We are currently in search of a Lead LVN to join our organization.

$75K - $100K DOE. 

Skills Needed:

• 2 years as a supervisor, manager or Lead in an Assisted Living, Memory Care, or Long-Term Care setting preferred.
• A registered nurse, licensed practical nurse or licensed vocational nurse licensed to practice as required by state regulations.
• Experience in direct patient care and assessments/observation of clinical conditions.
• Experience in managing people in the healthcare field preferred.
• Experience with employee scheduling preferred.

• Must have a hospitality mindset, as well as a customer service focused approach to care.
• Should have previous experience in a geriatric setting such as Assisted Living, Memory Care or Long-Term Care

• Experience at a department head level in Assisted Living or a related industry
• CPR certified and knowledge of basic first aid
• Experience coordinating educational events.
• Able to work flexible work hours due to demands of position.
• Position requires driving responsibilities, therefore you. must possess a valid driver’s license.
• Professional in appearance and conduct.
• Must be criminally cleared by DOJ and FBI via the Live-Scan process prior to beginning employment.

Responsibilities:

• Supervise, develop, and schedule the resident services staff according to Nazareth House policies, procedures, and standards.
• Maintain a medication system which adheres to state, federal and company laws, regulations, and guidelines.
• Perform assessments and reassessments to determine resident needs and establish appropriate service plans.
• Complete daily Quality Enhancement reviews to ascertain if the department is meeting state regulations and atria policies.
• Act as a contact for issues related to resident care within the community.
• Ensure adherence to company policy and State Health and Safety Codes, and federal regulations for assisted living communities.
• Schedule and supervise staff; available to cover staffing when needed.
• Administer medications as allowed per state specific guidelines when required.
• Provide training and orientation to new assisted living Care staff employees and ongoing training to current Care staff employees.
• Communicate to residents, families, department heads, resident services staff, and Executive Director in an effective and respectful manner as needed.
• Maintain positive relations with residents’ families and physicians.

Education Requirements:

Current and valid state certification as a licensed RN or LVN in the state of California.
